News : HRS CareConnect Supports Continuous Patient Care with New Clinical Monitoring Service
Hoboken, NJ, USA, Nov 29, 2017 - Health Recovery Solutions (HRS) announces the launch of a new clinical monitoring service to augment its telehealth solution. HRS CareConnect provides various levels of support, monitoring, and patient engagement. The solution gives providers the opportunity to deliver continuous care by supporting clinician coverage gaps to help minimize the risk of avoidable hospital readmissions and ED visits for high-risk chronically ill patients.

To deliver this program, HRS has partnered with Advanced Telehealth Solutions, a URAC-accredited health call center providing a broad range of clinical support services since 2002. With this partnership, the HRS CareConnect service will be available across the US to provide care interventions based on each provider’s unique clinical protocol.
